Adults on PEI living with Type 2 diabetes who do not have a primary care provider are invited to participate in a University of PEI (UPEI) research study evaluating a new approach to diabetes care. Conducted in partnership with Health PEI, the study will assess nurse practitioner-led group health visits as a way to improve access to team-based diabetes care. Participants will attend four 90-minute group visits over 12 months at UPEI. Each session will include a clinical assessment, education, and peer support. Participants should be comfortable in a group setting and have previously been seen by a member of the diabetes care team. At the end of the study, participants will be invited to share feedback in a focus group.
